Join TAKEheart

The TAKEheart initiative aims to increase patient participation in cardiac rehabilitation (CR) nationwide by promoting evidence-based strategies to improve CR referral, enrollment, and retention. Through expert-led web trainings, peer-to-peer engagement, and access to resources, TAKEheart supports participating hospitals as they move through their quality-improvement journeys. The initiative offers two ways for hospitals to participate.

Become a TAKEheart Partner Hospital

TAKEheart Partner Hospitals participate in a 12-month program focused on increasing CR by implementing automatic referral with care coordination. Join TAKEheart as a Partner Hospital to gain exclusive access to group-based technical assistance and individualized coaching.

Join the TAKEheart Learning Community

The TAKEheart Learning Community is a network of hospitals interested in exploring a broad set of strategies to increase patient participation in CR. Join the TAKEheart Learning Community to exchange information with peers about ways to increase CR referral, enrollment, and retention.

Join the Million Hearts® Cardiac Rehabilitation Collaborative led by the Centers for Disease Control and Prevention (CDC)

CDC’s Million Hearts® Cardiac Rehabilitation Collaborative (CRC) is a national forum of multi-disciplinary professionals taking action to achieve the national goal of 70% participation in CR for eligible patients by 2022. The CRC meets virtually to increase awareness about the value of CR, spread best practices, and build equity in CR referral and participation.
