The TAKEheart Team

The TAKEheart initiative to increase utilization of cardiac rehabilitation is made possible by the combined efforts of several organizations. Funded and overseen by the Agency for Healthcare Research and Quality (AHRQ), TAKEheart is supported by the Million Hearts® initiative and a contract team led by Abt Associates.

The three-year TAKEheart initiative to increase utilization of cardiac rehabilitation is funded and overseen by AHRQ as part of its continuing efforts to promote the delivery of evidence-based care.

Million Hearts®

A partner initiative, Million Hearts® is co-led by the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare & Medicaid Services to prevent one million heart attacks and strokes by 2022. TAKEheart applies strategies from the Million Hearts®/AACVPR Cardiac Rehabilitation Change Package, a quality improvement action guide developed by the CDC and the American Association of Cardiovascular and Pulmonary Rehabilitation.

TAKEheart Technical Expert Panel

The TAKEheart technical expert panel provides guidance on the initiative’s technical approach, promotes hospital recruitment and awareness building, offers input on evaluation activities, and reviews dissemination materials.
