Systems Change: Implementation, Teamwork, and Workflow Mapping

Overview

This module continues the systems change discussion with a focus on the work of the cardiac rehabilitation (CR) quality improvement (QI) team. Topics covered in this module include:

  • Mapping workflow and processes to identify necessary changes and potential barriers for implementing automatic referral with care coordination.
  • Collecting baseline data and establishing a data collection plan to drive decision making and improvement.

Suggested Audience

The target audiences for this session include:

  • The CR QI team
  • Non-team cardiac clinicians
  • Discharge planners
  • Care coordinators
  • Hospital leadership

It is important to note that the audiences listed above are a suggestion rather than a limitation. All are welcome to watch and review the module.

Learning Objectives

Upon completion of this module, attendees should be able to:

  • Map current workflows for CR referral, enrollment, adherence, and graduation.
  • Identify needed changes (when, where, by whom, and how) to implement automatic referral with care coordination.
  • Create a data collection plan that will inform your process and drive improvement.
